Apply for Enrollment

Thank you for your interest in enrolling your child at Mother of Hope Hybrid Academy.  Before you apply, please read our Statement of Faith.

If you wish to apply, you will need to fill out TWO forms.  Please fill out the Family Enrollment Application first (once per family), and then fill out the Student Application for each child you are interested in enrolling.

There is a $35 non-refundable application fee per family.  You may pay this electronically (incurs additional $1.50 fee):

Alternatively, you can mail a $35 check written out to: Mother of Hope Hybrid Academy.  Your application will not be complete until the check is received.  Please mail it to:

Upon acceptance, you will receive additional enrollment forms to complete, including a Code of Conduct, Dress Code Guidelines, Statement of Faith, and Financial Agreement.  Enrollment fees are as follows:

1st Child: $125
2nd Child: $125
3rd Child: $75
Any Additional Children: $50 each
