Apply for Enrollment

Thank you for your interest in enrolling your child at Mother of Hope Hybrid Academy.  Before you apply, please read our Statement of Faith.

If you wish to apply, you will need to fill out TWO forms.  Please fill out the Family Enrollment Application first (once per family), and then fill out the Student Application for each child you are interested in enrolling.

There is a $50 non-refundable application fee per family.  You may pay this electronically (incurs additional $2.50 fee):

Alternatively, you can mail a $50 check written out to: Mother of Hope Hybrid Academy.  Your application will not be complete until the check is received.  Please mail it to:

Upon acceptance, you will receive additional enrollment forms to complete, including a Code of Conduct, Dress Code Guidelines, Statement of Faith, and Financial Agreement.  The Enrollment fee is $125 per child for 1st-6th Grade, or $325 per child for Montessori (supplies and enrollment fee).
